Primary Care Pain Service

Dear Patient,

We have an optional new service starting within the practice aimed at helping support people with chronic or persistent pain that may be of benefit to you. The service is running throughout many GP practices in the UK and has already helped patients to manage their pain more effectively and work towards their own specific goals.

You are able to have regular sessions with a Health and Wellbeing coach who specialises in chronic pain. Throughout the sessions they will work with you to understand your pain and the complex nature of pain, what may be influencing your pain, and find strategies that work for you to help improve your symptoms. They will support you to identify goals that are meaningful to you, and help you to work towards reaching them.

Thes sessions are all over the phone or video call, so there is no need to travel. We will also be setting up a pain café or support group for people with chronic pain that will be a safe and relaxed place people can discuss what works for them, and help each other to manage their symptoms more effectively.

This is an optional offer of support and the service offers to help:

✔ Manage and cope with persistent pain

✔ Reduce the impact of pain on daily life

✔ Find ways to continue doing the things you enjoy

✔ Understand why chronic pain develops and persists
