About Tracey
Tracey Lightfoot is an enthusiastic and motivated individual who is a fantastic support to our Primary Care Network and member practices in supporting with Immunisations. Tracey also has a clinical nursing background that helps to support her fulfil this role.
Aims of the role
To support our member practices increase the uptake of childhood immunisations and promote NHS immunisation campaigns.
The process and what to expect as a patient
Each day, our Immunisation Coordinator will contact the parents of children who are booked into the clinic for the next day to remind them of their upcoming immunisation appointment.
If the parent confirms the appointment, the Care Coordinator will add confirmed with the clinician that the patient will be attending the appointment.
If the parent cannot make the appointment, the Care Coordinator will reschedule for another day or ask the parent to contact the practice to rearrange the appointment if they cannot reschedule at that time. The Immunisation Coordinator will cancel the appointment.
If the Care Coordinator cannot speak to the parent, a voicemail is left stating the appointment date and time and informing the parent to contact the practice to reschedule if they cannot attend; if voicemail is not available, an SMS is sent with the appointment date and time with instructions on how to reschedule.
