About Courtney

Courtney Dale is an enthusiastic and motivated individual who is a fantastic support to our Primary Care Network and member practices in supporting with all Care Homes. Courtney has a strong primary care background in Care Homes, Vaccinations for Housebound Patients. Courtney acts as Lead Care Coordinator, and oversees work and projects completed by our friendly Care Coordinator team.

Aims of the role

To support and guide our care coordinator team with their day-to-day roles and to plan and support the organisation of Care Home Personalised Care Plans and the delivery of housebound vaccinations, liaising with outside stakeholders and other members of our PCN visiting team.

The process and what to expect as a patient

 

Courtney works off a schedule attending Multidisciplinary Meetings (MDT) alongside a Care Home Lead GP in each care home, Enhanced Care Home Support Team (ECHST) to discuss Care Home patients advanced care planning and arranging any further discussions that may need to be had with the patients next of kin and family members. 

Courtney also completes the registration process for our care home patients to ensure that all relevant teams are aware of any new admissions so appropriate reviews can be completed by our clinicians.  

About Lauren 

Lauren Mccarroll is an enthusiastic and motivated individual who is a fantastic support to our Primary Care Network and member practices in supporting with all of our additional services and appointments that we offer in our ‘Hub’. Lauren has a strong primary care background, and has previously worked as a Anima triage coordinator and Medical Receptionist at a GP practice in Warrington.

Aims of the role 

Warrington Innovation Network offer additional services and appointments such as Advanced Nurse Practitioner Appointments, First Contact Practitioner Appointments, Prescribing Psychology Appointments, Medication Review Appointments and many more. Lauren is in charge of coordinating our appointments, booking patients in, rescheduling, and communicating with patients with changes of appointments. Lauren also supports administrative work such as completing administrative work for clinicians.

The process and what to expect as a patient

If you have booked an appointment via your practice for one of our many services, Lauren will send you an appointment reminder 24 hours before your appointment. At this stage, you have the chance to cancel your appointment should you no longer be able to attend.

If a clinician is unavailable, and we need to cancel your appointment, Lauren will communicate with you directly either via SMS or phone call and will re-book your appointment.

At the request of your GP practice or clinician, Lauren may contact you to book you in for a Medication Review, Home Visit, Vaccination or Follow up appointment.

About Tracey

Tracey Lightfoot is an enthusiastic and motivated individual who is a fantastic support to our Primary Care Network and member practices in supporting with Immunisations. Tracey also has a clinical nursing background that helps to support her fulfil this role.

Aims of the role

To support our member practices increase the uptake of childhood immunisations and promote NHS immunisation campaigns.

The process and what to expect as a patient

Each day, our Immunisation Coordinator will contact the parents of children who are booked into the clinic for the next day to remind them of their upcoming immunisation appointment.

If the parent confirms the appointment, the Care Coordinator will add confirmed with the clinician that the patient will be attending the appointment.

If the parent cannot make the appointment, the Care Coordinator will reschedule for another day or ask the parent to contact the practice to rearrange the appointment if they cannot reschedule at that time. The Immunisation Coordinator will cancel the appointment.

If the Care Coordinator cannot speak to the parent, a voicemail is left stating the appointment date and time and informing the parent to contact the practice to reschedule if they cannot attend; if voicemail is not available, an SMS is sent with the appointment date and time with instructions on how to reschedule.

About Leanne 

Leanne is an enthusiastic and motivated individual who has a strong pharmaceutical background (non-clinical), with experience in minor ailments and patient signposting.
 

Aims of the role

To support our member practices increase the uptake of patient signposting to alternative services such as District Nurses, Pharmacy minor ailments, First Contact Practitioners and various other services ensuring patients get the Right Care, First Time.

Leanne is currently training in Screening including Bowel Screening, Breast Screening and Cervical Screening. She aims to support our member practices with uptake of screening, and to support patients with the screening process.

Leanne also currently supports patients who are registered with their practice as a ‘Carer’. In 2025, Leanne will be offering carers assessments to any patients registered as carer, to ensure unpaid carers are receiving the help and support they need.

The process and what to expect as a patient

As you may be aware Warrington Innovation Network promotes the ‘Right Care, First Time campaign’ this ensures that patients are triaged and given the ‘Right Care, First Time’ without necessarily needing to see a GP every time. If you are triaged by Leanne, you may be signposted to alternative services.

As Leanne is working on Screening and Carers Assessments, you may receive an SMS message with further information, or to confirm if you are still registered as an unpaid carer. It is important you respond to these messages so your medical records are updated.
